The OAS Medical ID Card, initiated by the Ostomy Association of Singapore (OAS), is designed to provide medical identification and facilitate smoother interactions for ostomates living in Singapore. Our card serves multiple purposes:
Travel Convenience
Simplify communication with transportation security personnel and airline flight attendants, especially when you prefer or need to communicate non-verbally. The card helps convey essential information about your medical condition, ensuring a more comfortable and hassle-free travel experience.
Restroom Access
Gain easier access to handicap toilets for ostomy bag changes. The card communicates your need for accessible facilities, allowing for discreet and respectful use of restroom amenities.
How to apply?
For existing OAS members, please complete this form:
•Photo minimum size : 3.5cm(w) X 4.5cm (h), Resolution: 300 dpi
•Photo is acceptable as long as the facial features are clearly visible
For new members, please submit a membership form at E-Form – Ostomy Association of Singapore (oas.org.sg) and indicate that you would like to receive the Ostomy ID card and email your photo to admin@oas.org.sg
You will receive your ID card within two working weeks of application.
